These are generic airliner answers as I don't know that aircraft:

  • Most likely if you did nothing unusual and both engines were affected, is icing. If the outside air temperature is lower than 5°C and you didn't activate any anti-ice systems, ice can be a problem.
  • X-Plane has detailed failure options (see Aircraft / Equipment Failures), which default to no failures, but custom aircraft can have their own additions to this: if you do a lot of flights without doing certain maintenance tasks, the aircraft can remember and fail a component. Check your aircraft's manual.
  • It could have been associated with some other failure. Did you turn on the generators after engine start? Without them (incl. APU gen) an aircraft's batteries can eventually run dry, leading to deployment of the Ram Air Turbine; I don't know if this state can also cause any engine trouble? You would likely see reduced cockpit displays in the event of electrical failure - but it's much more likely that engine failure causes loss of electrical power than the other way around, to be honest.
  • It's also possible to accidentally hit a key bound to fuel cut-off or some such.

Well done thanks I wondered how the little fan under the belly of the aircraft deployed..lol, makes quite a noise, and 2nd ice was over looked.
